Funeral services for Velma Forman, 66, will be held Sunday, January 21, 2007 at 2pm in Calvary Baptist Church in Bayou Chicot. Burial will follow in the Vandenburg Cemetery. Rev. Leon Terrell will officiate the services.

Mrs. Forman, a retired registered nurse and a resident of Bayou Chicot, died Friday, January 19, 2007 at 1:15pm in Ville Platte Medical Center.

Survivors include her husband, Ray Forman of Bayou Chicot; her step father, Gorman Weldon Smith of Silsbee, Tx; one son, Garland Ray Forman, Jr. of Bayou Chicot; two daughters, Patricia Bagwell of Pineville and Rebecca Crilly of Mandeville; one brother, Charles Koepp of Tx; one sister, Rose Ann Thibodeaux of Silsbee, Tx; nine grandchildren, eleven great grandchildren, and numerous nieces and nephews.

Visitation will be held at Calvary Baptist Church in Bayou Chicot on Saturday, January 20, 2007 from 2pm 10pm and on Sunday from 8am until time of services.
